Enroll your kids in the Chuck E. Cheese Birthday Club today and they'll receive special gifts during their birthday month! All messages are sent to the parent. Must enter valid phone and email address to receive benefits. Kids must be 17 or younger to participate in the Birthday Club. Only adults may fill out this form.
